Corporate Integrity and Compliance Plan

As part of Community Counseling Center's commitment to integrity in our services, this form may be used to anonymously report concerns regarding Waste, Fraud or Abuse that you may have witnessed or experienced by the Corporation and its employees. You may also anonymously call the Director of Human Resources/Corporate Compliance Officer at 440-998-4210, ext. 253 to make a report.

Corporate Compliance Plan:

• This Plan evidences CCC’s long-standing and continuing commitment to ethical business practices and to compliance with all applicable laws and standards governing the delivery and billing of mental health, alcohol and other drug services to its clients and patients, and the maintenance of the confidentiality of client/patient service records.
• This Plan has been designed to ensure that the delivery of services by CCC is in accord with the highest standards of the mental health services industry and that those services are properly documented and accurately billed to CCC’s clients, patients and/or the health care insurance programs in which CCC’s clients and patients are enrolled.
• This Plan has also been designed to prevent, detect and correct instances of non-compliance with applicable laws, standards and the conditions of participation of private and governmentally-funded health care insurance programs.
• CCC is committed to a regular and vigorous program of employee training on the contents of the Compliance Plan and the policies and procedures through which it is implemented.
• It fully expects that all of its employees and agents will adhere to the Standards of Conduct set forth in this Plan and that any deficiencies in the design or implementation of the Plan, as well as any violations of its Standards of Conduct, will promptly be reported to management or the Plan’s Compliance Officer.
• CCC also expects that those persons and entities with whom it contracts for goods or services, or from whom it receives referrals for services, will, to the extent applicable, adhere to this Compliance Plan. CCC will, therefore, ensure that all contract employees, vendors of goods and services and referral sources are informed of the Compliance Plan and its applicable components, agree to abide by its provisions and are updated.
